The problem
Patients already own their records. Almost none of them can actually use them.
A patient's history is scattered across a primary care chart, a cardiologist's notes, a dermatology biopsy, a dental panorex, and three labs that each have their own portal. Every one of those is legally theirs. In practice it is unreachable, and so is the context that would make an answer about their health worth anything.
Assembled
Records come in through the standards clinicians already publish, then get normalized into one longitudinal file that spans every practice a patient has seen.
Attributed
Every value keeps its origin: which organization, which clinician, which encounter, which date, and whether it was entered by a clinician or reported by the patient.
Answerable
The patient connects the assistant they prefer. Questions get answered against their actual history rather than against generic medical writing.

Structured facts stay structured
When a patient asks for a lab value, a dose, or a diagnosis date, the answer is retrieved from the underlying clinical record rather than summarized out of prose. Narrative notes add context and are labeled as narrative.
Every claim carries a citation
Answers point back to the source record, with the organization, author, and date attached, so a patient can take it to an appointment and a clinician can verify it in seconds.
Disagreements are shown, not resolved
When the medication list and the visit note conflict, both appear with their sources. Deciding which is medically correct is a clinician’s job, and the software does not pretend otherwise.
The assistant is the patient’s choice
Claude, ChatGPT, or a custom agent. A patient’s medical history should not be locked to whichever AI vendor happened to build the connector.

MyWholeFile MD can never write to a chart, place an order, send a message on a patient’s behalf, or alter a clinical record. That is not only our policy. The underlying interfaces we use are read-only, so the capability does not exist to be misused.
